Please give a referral letter or a completed referral form to your patient and ask them to contact our practice to book their appointment. A copy of the referral can be emailed to reception@eyeandretina.com.au or faxed to (02) 9699 0002. This helps to ensure the referral is triaged and booked appropriately when the patient contacts us.
